Our Vision:
We envision a long term care continuum where individuals are supported and cared for in ways that respect what is important to them and for them.
Triangle J Area Agency on Aging (TJAAA), located in central NC and serving Chatham, Durham, Johnston, Lee, Moore, Orange and Wake counties, has been spearheading an initiative within the area to raise awareness of the need for more person-centered approaches to care and services for older and disabled adults throughout the long term care continuum, in lieu of the institutional model of service delivery that is currently more prevalent. This shift towards person-centered thinking is motivating us to create more home-like environments within our institutional settings and service delivery systems, where the needs and desires of older adults are recognized and the safety and comfort of “feeling at home” is evident.
Changing the institutional mindset of the past necessitates fundamental changes in the way that organizations plan and deliver care and services for our frailest citizens. This new way of thinking challenges everything about the way that our current system works and the staff of Triangle J Area Agency on Aging are leading the transformation effort in the local area through their ongoing “Path To Home, Sowing the Seeds of Culture Change” initiative. This blog chronicles the thoughts and insights gathered along the way and seeks to create a virtual learning circle where many more can benefit from the collective wisdom of those who share in our broad vision.
